This card is NON-TRANSFERABLE.

SMO will not be held responsible for stolen, lost or damages cards. An administrative fee of $15 shall be levied on each replacement of the membership card.

Card is valid for 1 year from Date of Registration.

SMO reserves the right to amend or withdraw any part of the benefits & privileges, terms & conditions of the membership without prior notice.
